TEXAS – The Dallas County Department of Health and Human Services explained what to do if you got vaccinated against COVID-19 but lost your card.
Through a message on its Twitter account, the department indicated that if you need to obtain proof of vaccination they cannot give you a new card, but you can obtain a copy of your record.
All you have to do is call 972-692-2780 Monday through Friday, 8:30 am to 5 pm
